The Physical Form can be found in the Medical section of the Athletic Clearance process. You must complete the Student Information and Parent/Guardian Information pages before you will be able to access the Physical Form.
The Physical can be found at the bottom of the Medical page. Click on the Paperclip Icon to download the attached file.
The Physical will also be available on the Files upload page. Please note that you will need to complete all preceding sections before you can access this secion.
